《蝙蝠侠 阿甘之城》仍然建立在《阿甘疯人院》的气氛上，不过这次上升至阿甘之城——高谭市内戒备森严的，关押了大量暴徒的监狱之中。而且新作还汇集了众多明星参与的配音阵容以及蝙蝠侠中的极度凶残的恶棍，并改进和加强了一游戏特点，让玩家们拥有像《蝙蝠侠前传2：黑暗骑士》一般的终极游戏体验。
BAC的故事紧接前作，在专门关押恶棍的阿甘疯人院被镇压之后，典狱长Quincy Sharp将功劳全部归于自己，以此赢得了接下来的市长大选。他上任后的第一件事情，就是购买了城中大片的贫民窟，将其改造为一个规模更大的巨型监狱，这就是阿甘城。当年阿甘疯人院的那场灾难让Sharp记忆犹新，于是他为这座监狱创造了一个全新的，同时也是唯一的规则：“不要尝试逃跑”——只要不越狱，在里面干什么都可以。监狱的戒备由嗜血的Tyger雇佣兵担任，其实他们只负责驻守阿甘的“城墙”，射杀一切想出去，或者是未经授权的进入人员。在前作中重伤的小丑被Harley Quinn治好，他准备再度掀起破坏活动。与此同时，疯博士也被送进了监狱，不甘寂寞的他让这座恶人集中营中关押的犯人陷入了极端的狂暴之中。
